Here is a(nother) freebie for you. You could use it for anything. Print it front/back and fold it in half. They write three clues on the front and draw a picture and fill in the blank on the inside. Then they cut the dotted lines to make flaps. So as you read the clues you can open up to get a visual clue too. Hope that makes sense!
